What is an FQHC?

River Valley Health is a Federally Qualified Health Center, a nonprofit community-based health organization. An FQHC provides comprehensive primary health care, dental care, and mental health services to individuals of all ages. Also known as Community Health Centers, FQHCs use a sliding fee scale program to ensure that every person has access to care, regardless of their ability to pay.

Its state-of-the-art facilities and equipment, highly qualified staff, and commitment to affordability are some of the standout features that make us not only an excellent healthcare provider, but also a great place to work.

Healing together through Addiction Medicine

We recognize the challenges of navigating recovery, and we are committed to offering comprehensive support through our Medication-Assisted Treatment (MAT) services. Our team of experts at River Valley Health diligently develops personalized care plans to empower you on your recovery journey. We prioritize your well-being, ensuring you have the necessary tools, access, and knowledge to effectively manage your recovery.

Patients enrolled in MAT services are supported by their provider and the MAT nurse care manager. This dedicated team works together alongside the patient to develop individualized treatment plans, recognizing no two journeys are alike. Through routine appointments and regular care management services, support, and education – patients are connected to resources to aid them in their recovery. The MAT program includes access to various medication assisted treatment options including oral medications and injectables that can support treatment of opioid use disorder and alcohol use disorder.

Diabetes Management ~ A path to a healthier future

We understand the challenges that come with managing diabetes, and we are dedicated to providing comprehensive support to all patients. Our team of experts work to create personalized care strategies that empower you to lead a healthier life. At River Valley Health, we prioritize your well-being, ensuring you have the tools and knowledge to effectively manage diabetes and thrive.

We refer patients with diabetes to an in-house certified Diabetes Educator and nurse care manager for self-management support and education. This team focused approach allows patients to remain at the center of their care and gain necessary knowledge to manage their diabetes and improve their overall health outcomes.

Using Evidence Based Curriculum, our Diabetes Self-Management Education and Support (DSMES) program provides education around healthy eating, healthy coping, being active, taking medication, reducing risks, problem solving, and monitoring glucose. This includes the use of continuous glucose monitors (CGM). The entire team works together with each patient to develop an individualized approach to glucose control.
