Protecting Patient Privacy Matters
As a volunteer with River Valley Health, you may see, hear, or have access to confidential patient, employee, or organizational information while performing your volunteer duties. The Health Insurance Portability and Accountability Act (HIPAA) is a federal law that protects the privacy and security of patient health information.

River Valley Health is committed to safeguarding the confidentiality, integrity, and security of all protected health information (PHI). Every team member of our health center, including volunteers, are responsible for maintaining patient privacy and handling information appropriately. Failure to comply with HIPAA requirements may result in removal from volunteer service and could carry legal consequences.

Confidentiality Acknowledgement

I will respect the privacy concerns of the people we serve. I will hold in confidence all information obtained in the course of professional service, whether the information is obtained through written records or daily interaction with the person. Therefore, I will not disclose an individual’s confidences to anyone except: (1) as mandated by the law; or (2) where I am compelled to do so by a court as pursuant to the rules of a court.

I agree to respect absolute confidentiality in regards to any patient that enters River Valley Health. I will not read a patient’s chart without expressed authorization, nor will I discuss clinic matters with anyone outside of the clinic.

I will store or dispose of professional records in ways that maintain confidentiality.

I will possess a professional attitude which upholds confidentiality toward the people we serve, colleagues, applicants and any sensitive situations arising within River Valley Health.

Upon termination of my employment/relationship at River Valley Health I will maintain client and co-worker confidentiality and I will hold as confidential any information about sensitive situations involving River Valley Health.

I understand that violation of this confidentiality statement may be grounds for immediate dismissal from employment, and/or legal action.
